All returning student-athletes to Penn State Altoona must complete and submit the following two forms to Penn State Altoona Athletic Training prior to the start of practice and competition:
This packet contains information such as emergency contacts, insurance information, consent for treatment by ATC, and assumption of risk.
This form can be completed by your family physician or the Penn State Altoona Health and Wellness Center. Returning students with an activated user ID (ex: abc1234) may schedule an appointment at the Health and Wellness Center online at altoona.psu.edu/health-wellness/online-access.
This physical form helps to make the sports medicine team and head coach aware of any pre-existing injuries or medical conditions. The certified athletic trainer may contact you if you recently had any surgery, have a pre-existing medical condition, or were diagnosed with a new medical condition to request additional clearances by your surgeon or medical specialist.
The sickle cell test is required for all NCAA Division III student-athletes. This test is an out-of-pocket expense for the student-athlete. It is not covered by Penn State.
**If you do not have health insurance coverage, you must submit one of the following affidavits to Penn State Altoona Athletic Training:
Affidavits must be notorized. This can be done at the Switchboard at 101 Smith Building on campus. The Switchboard can be reached at 814-949-5000, and its office hours are 8:00 a.m. - 5:00 p.m. Monday through Friday.
