All student-athletes who have not previously played an NCAA Division III sport at Penn State Altoona (including incoming freshmen, first-year newcomers to a team, and transfer students) must have the following two forms completed and submitted to Penn State Altoona Athletic Training prior to the start of practice and competition:

1. Sports Medicine Packet (found in SportsWare)

This packet contains information such as emergency contacts, insurance information, consent for treatment by ATC, and assumption of risk.

2. Physical Exam Form / Sickle Cell Solubility Test Results (found in SportsWare)

This form can be completed by your family physician or the Penn State Altoona Health and Wellness Center. Appointments can be made at the Health and Wellness Center by calling 814-949-5540. No walk-in appointments! New students can be accommodated for physicals during SOAR (Student Orientation, Advising, and Registration) when they are on campus.

This physical form helps to make the sports medicine team and head coach aware of any pre-existing injuries or medical conditions. The certified athletic trainer may contact you if you recently had any surgery, have a pre-existing medical condition, or were diagnosed with a new medical condition to request additional clearances by your surgeon or medical specialist.

New Criteria starting FALL 2025:
The sickle cell solubility test is required for all NCAA student athletes. This test is an out-of-pocket expense for the student athletes. It is not currently covered by Penn State. If you were born in the state of PA the newborn sickle cell testing DOES NOT MEET the criteria set forth by the NCAA. Currently this is the only outlined state that doesn't not satisfy the NCAA criteria. All other states sickle cell newborn testing will be accepted and must be uploaded to the student athletes SportsWare profile as part of their preparticipation requirements. If you were born in PA, then you must obtain a lab script for the sickle cell solubility test from your medical provider, complete the bloodwork and upload your test results to your SportsWare profile as part of your preparticipation requirements. Please give labs 2-3 business days for results.
